Community Services

Our specialist teams provide person-centred, individualised, high-quality and responsive care for local people living with any life-limiting illness. Through specialist and compassionate care, we support patients and those closest to them, empowering them to enhance their lives, to live as well as possible, for as long as possible.

Living with a life-limiting illness can be challenging for patients, as well as those closest to them too. Through supportive and personalised care, our Wellbeing and Community teams empower individuals and their carers to take positive steps towards managing their physical and emotional wellbeing.
