As a team within the Hospice’s Community Services, our Therapy team provide advice and support on symptom management with an aim of improving quality of life and reaching meaningful goals.

The team supports patients across the Hospice and in patients’ own homes to address challenges they may face with mobility, daily tasks, and wider symptom management whilst encompassing psychological, social, and spiritual aspects of the holistic care we offer. Through working closely with other teams, both internally and externally, to meet the complex needs of patients and their caregivers, the team aims to support individuals to maintain their wellbeing and independence.

THERAPY SERVICES

Our Physiotherapists and Occupational Therapists provide support, interventions and education in one-to-one and group sessions, both in the community and at the Hospice in the Therapy Outpatient clinic. For those on the Inpatient Unit, the Therapy team works closely with the Inpatient team, caring for patients with complex symptoms, those awaiting discharge and those at the end of their life.

Based at Willow Wood Hospice, the Therapy team is available 8.30am-4.30pm, Monday-Friday and can be contacted on 0161 674 9787.

THERAPY SUPPORT

Through regular scheduled appointments and continued assessment, our Therapy team supports patients with:

• Day-to-day tasks
• Mobility
• Moving and handling advice
• Rehabilitation, including bespoke exercise programmes
• Home environment adaptations and equipment assessments
• Breathlessness, anxiety and fatigue management

HOW TO ACCESS OUR SERVICES

Our therapists care for adults aged 18 or over who are either a resident within, or registered with a GP in, Tameside or Glossop, and who have been diagnosed with a life-limiting illness.

The levels of care and support provided by the Therapy team is based on a holistic assessment, completed by a qualified therapist, at point of referral.

Therapy Support is accessed via a referral from a GP, other healthcare professional or via accessing other Hospice services.

We care for everyone, regardless of gender, disability, race, religion, sexual orientation and diagnosis and we are proud to be recognised as a ‘Homeless Friendly’ hospice.
